Install issue - OS 9 on SCSI Drive

Please help! A friend of mine has a Power Mac 9600 w/ a Sonnet Crescendo card. He asked me if I could help him out with the installation of a new hard drive and OS install. The old hard drive is no longer usable. I am tying to install OS 9 on a new SCSI drive. This is the only hard drive in the machine. It is terminated correctly. I am using the 9.0 apple install disk. Everything starts ok, but then I get the following error message: Problems were encountered reading the “Source file installation tome” installation cannot continue. Other than trash the machine or give up, anyone know a solution or work around for this issue? Thank you -

check cd install disc, and cd reader, to be cleaned, check connections/wires, try again... is drive formatted hfs+
try different cd reader to install with,

That will without a doubt be either: The cdrom drive or the CD itself. It's not the mac as it does see the cd and it's already formatted the hard drive.
