That might be a winmodem. Crative does not make Linux drivers for their modems, and AFAIK they won't (according to their site a while back when I was looking for a driver for another Creative modem).
You can try linmodems.org and see if there is one written by a user for the modem you have.

Yeah you're right, it is a winmodem. To make matters worse, the chipset does not seem to have any linux support.
Since Mandrake did could not even detect the modem to identify the chipset, I booted up Knoppix, which, surprisingly, was able to identify the chipset as an ESS 2898 Modem from Digicom Systems.
I looked that up on the PCI Modem databases and it appears to lack support from linux. So I gather my modem is as "win" as they get...
