I have a machine running E-smith 5.6 (Redhat without all the GUI, only server related RPMs).
The server is behind a Netgear router. Port 80 is forwarded to it. But it doesn't seem to want to respond to connection.

I assume Apache works fine behind the LAN. If you have any doubts, from another computer behind the firewall type in the Apache server's IP in a web browser.
"Port 80 is forwarded to it."
Forwarded to port 80 on the Apache server? When you forward, you have the option of accepting the external input on one port (in this case port 80) and sending it to an internal host (in this case the Apache server) on another port (maybe 8080, for example). Be sure Apache is configured for the internal port to which you are forwarding incoming TCP requests (make sure the router is set to forward TCP specifically while you're at it).
"it doesn't seem to want to respond to connection."
The forwarding is completely unknown to the server (it is masked and handled by the router). So assuming this is a default installation of Apache (more or less), the server either accepts and returns HTTP requests or does not.
