Although not exclusive, the most likely culprit is the device driver. Look on the manufacturers web site for the most update driver. If that doesn't work then the less likely culprit would be a device/hardware conflict which means that two or more devices are attempting to use the same resources such as memory space or port address/DMA assignments. After that you might look into whether or not the drive itself is faulty.
