Cable modem = Cable IE: Thin net cbaling like for your cable TV service. from the cable modem to the PC is either CAT5 network cabling or USB depending on the cable modem you have.
DSL = Digital Subsci... Line = A dedicated copper analoge line. This would be a phone line with some modifications to it depending on the provider. From the DSL modem to the PC is the same as above. Either CAT5 network cable or USB depending on the DSL modem.

For which one?
for Cable the cable company has to come out and install a line, put in regulators and filters and such at the poll.
For DSL they have to come out and test the line between your house and the CO to see if the line can handle the data rate and such. For most DSL connections you must be within 18,000 feet from the CO, and the DSL company must install hardware on the line so that it can be used for DSL.

not usually. You'd plug an adapter/splitter/filter into the phone line jack, then run one line to your phone and the other to your DSL modem.

The phone company will do what is needed to bring the digital service to your home phone lines. The DSL modem simply plugs into your standard phone jack (no filter). If you arent provided with filters, then purchase some to use on phones,fax machines, etc plugged into the other jacks around your home. These filters protect the analog devices from damage which can be caused by the digital signal.
The other hookup is normally a network cable from the DSL Modem to a NIC card installed in the computer. As mentioned above however, This may be done using the USB ports as well. Good Luck
