Everytime i insert a USB device into any USB port, the mouse freezes, and the system crashes, have to do a hard-reset.
My OS is XP home sp2 and entirely updated excluding sp3,
In 'Device Manager' all entries are described to be working fine except for 'SM Bus Controller' for which i cannot find the driver for,
However, i am not too sure if this relates to my problem, so if someone could help and perhaps insight me into what this is exactly and what it is for just to extend my computer knowledge,
That aside providing it is not causing the problem my pc seems to work ok with that missing so my main problem is that concerning the USB so any feedback on that especially would be much apprieciated.

Start at event viewer logs. If nothing see how to config dump logs and read them.
For most times a driverless device doesn't cause a crash. It just doesn't work.
Even though mouse freezes, can you access via tab and alt tab or other hot key combo's?
"Best Practices", Event viewer, host file, perfmon, antivirus, anti-spyware, Live CD's, backups, are in my top 10

Nothing is being logged in event viewer,
I retried to connect a usb device and work via keyboard like you suggested but unfortunately my keyboard locks up too, num lock sticks and all keys/hotkeys are non active, (tab, alt-tab, ctrl-alt-del, etc)... nothing
I tried to uninstall the USB controller devices and then restart to scan for hardware and when it restarted and began to pick up and install detected hardware it was freezing up again as soon as it was trying to install the driver for USB root hub, it was doing this with no USB device inserted, i surpassed this via safe mode and they are again now present but unusable,
