Overclocking Video Card

Hi. I am a little confused about overclocking...so please help. I have a Geforce FX 5700 Ultra and I've overclocked this card many times before. Whenever I auto-detect it to a certain memory/core clock the graphics look like its been *ripped apart* or something, and I know its because of the o/c. The thing is I used to set it to high memory/core clock and it worked fine, but now it seems as if it is getting weaker ;more less stable when it comes to overclocking.
Any suggestions? If I get a new fan for my gpu will it help at all?

Check the fan and heat sink for dust build up. Your fan may be flowing well but no air moving do to dust build up. Could also reinstalling the drivers.
