Move DC\Exchange to new Hardware

I need to move my current DC and Exchange wich is running on one hardware to 2 new hardware. After i repaired the old hardware i will add it as an extra DC. I want to keep the same IP and Domain name. Can anyone provide me with the steps to do this.
