If using a Front or Bracketed USB port my guess would be the port is improperly wired to the motherboard. This is possible even with single connectors. Gigabyte, for instance uses a different pinout than everyone else.
To eliminate that as a pooible cause, use USB ports that are integrated onto the motherboard.
