It means that some app is using memory and not returning it after its done with it. That means that in the long term (hours? days?) its allocated memory level will be higher and higher, and it could cause a crash when it reaches a critical level. When it reaches this level depends on how fast it is leaking. If its a server you have a problem, if its a home computer and the leak is slow, you can probably live with it. Its like oil in a car. You fill it up every now and then, and the same way you reboot your computer to reset the "memory".
