Is my video card bad?

Specs: Windows XP, Intel 2.8g / I gig ram
I was playing sims 2 last year when suddenly I get these lines all over my screen. I restart and it wouldn't go away. I've tried replugging my video card in and reinstalling the drivers but nothing works. Now the green lines will change colors depending on the screen I am on. I also see weird display when the computer is starting up so it's not just in window. Is my video card or my motherboard going bad??? I don't think it's my monitor because I tried plugging my monitor on another computer and it works perfectly. Thank You so much

Video Card - ATI Radeon 9800PRO 128MB

Sounds like the video card. It's an oldie but a goodie. Make sure the 4-pin molex plug is connected to the card.

Yea, it was a very good video card, is the 4 pin molex plug the power cord? There is power to the video card so that means it's good right?

What I'm asking is, is the 4-pin plug connected to the back end of the card? You can see it in this pic:

no, I don't think so, how do you connect it? The white plug with the four wires(red, black, black and yellow) is connect, but I dont see anything on the other side of the card that has anything to connect. (Sorry, I am not that good with computer)
