Your power supply does not have enough amperage on the 12volt rail because it has 2 12Volt rails and not enough amperage on either, in fact it it would take both of your rails to supply barely enough for full load but that would not happen. Further your card requires two 6pin connectors and yours only has one (an adapter is not the same).
Cooler Master 500W PSU Extreme Power Plus
Cooler Master 500W PSU Extreme Power Plus
Form Factor: ATX
Wattage: 500W
Fan: 120 mm
+3.3V: 22 A
+5V: 25 A
+12V 1: 18 A
+12V 2: 18 A
-12V: 0.5 A
+5VSB: 2.5 A
20+4-Pin Connector: 1
4-Pin/8-Pin EPS Connector: 1
6-Pin PCI-Express Connector: 1
4-Pin Floppy Connector: 1
4-Pin Peripheral Connector: 5
SATA Power Connector: 4

Roughly yours:
GeForce GTX 460 (Fermi) 768MB-1GB 192-256bit GDDR5
Core Clock: 675mhz
Shader Clock: 1350mhz
Cuda Cores: 336
Effective Memory Clock:
Power Consumption at Idle: 29 watts
Power Consumption at Full Load: 180 watts

The recommended power supply is 450 watt with two 6 pin PCI Express connectors. For SLI 650 watt power supply is recommended.
Recommended 36 Amps on the 12 volt rail, 50 Amps for SLI.
Purchase a power supply with at least 500Watts (600 preferred) with at least 36Amps on a SINGLE 12 volt rail AND at least 80% efficient rating to run that card properly under FULL load.
Then IF it still has problems, return the card for replacement.

You have to be a little bit crazy to keep you from going insane.
