Frequency Out of Range

About a week ago i got a new asus radeon 9600 xt vid card. I was playing return to castle wolfenstein on the comp today and twice it has happened where the comptuer would lock up and the monitor would lose a signal from the computer and just say "frequency out of range." I checked to make sure the refresh rate is fine for the monitor with the resolution I am using and it was. 85 refresh rate at 1024x768. I am using the latest ati drivers as well as the latest dell drivers for my monitor. What could be casuing these seemingly random errors?

It seems that the graphics card is intermittently not sending a signal to the monitor. This might be a heat problem, faulty cable, or defective graphic card.
