floppy disk problems

My floppy disk drive is unable to read or write to floppy disks. Under windows I get the disk is not formatted error (and it is unable to format the disk if I try). In ms-dos I get an invalid media type error. In safe mode the drive is not accesible. Even scandisk is unable to check drive A; because of the disk is not formatted crap. The disks I have tried are all preformated 3 1\2 inch floppies. I have tried different brands. The cables seem to be fastened tightly and the floppy led comes on. There are no conflicts and the driver is working properly. Does anyone know whats causing this?
