You used a floppy to create the bootable CD. On that floppy you have to set up config.sys with a line to load USBD.sys. It appears that the line (22) was not setup correctly. It either has a typo or is not pointing to the correct location for USBD.SYS.

The other error is a result of the first error.

I don't know what to tell you, except that line 22 in the config.sys file on the CD that you created is wrong. Check the config.sys file on the CD and make sure it points to the location of the USBD.SYS file.

I have seen this behavior also when trying to create a CD that will boot from USB. What I believe is happening is that as soon as the USB CDROM driver is loaded (probably on line 21 of config.sys), this driver takes over USB controller function and overwrites any of the legacy USB device support (floppy, keyboard, mouse, etc.) that is supported by BIOS. This causes drive A: used by the floppy emulation portion of the CDROM boot to disappear. Because drive A: is now missing, config.sys (the text from config.sys are still in memory) cannot find the file USBD.SYS defined in the device= command. I am still trying to solve this problem. Microsoft seems to have a solution becuase I can use the XP system CD booted from a USB CDROM to restore my hard drive and still have access to the mouse and keyboard. I wish they would share their method of doing this.
