Depends on your definition of PC.

3D Monster Maze on the Sinclair ZX81 back in 1981, if you class the ZX81 as a PC.

If you're talking about modern(ish) PCs, it'll be Wolfenstein 3D.


Some people are like Slinkies, not really good for anything, but you still can't help but smile when you see one tumble down the stairs...

Actually Ultima Underworld was released one week before Wolfenstein.
