Are you reading that number off a chip or is that printed on the board? Every search I do comes up with chips with that number.

You need to check the larger numbers printed on the board or on any stickers it may have. The ones on the chip are for that chip only.

Check this link out. Scroll to the bottom and take a look at w6692. I belive that this will work. Compgeeks sold this modem at one time and I think that I used these drivers.
