I would get a new fan and install it. If the card is under warranty they will replace it for free (if you want to go that route)
Buying a fan is easier and faster.........

Buying a replacement fan would be your best bet. In the meantime, clean the fan completely with a fine brush and some compressed air. You can add a single drop of fine machine oil to the bearing of the fan by peeling up the decal in the center of the fan. Allow the drop of oil to soak into the bearing and use compressed air to spin the blades to help the oil coat all the bearing surfaces. Stick the decal back down and reinstall the video card.
