If it is a thermal pad, you should rep[lace it each time you remove the processor/heatsink. If it is paste and still in good condition you can get away with not, but I would still recommend applying new paste.

It depends on the re-installation interval, is it days, weeks or months apart? Look at the HSF and use your judgement - even if it's minutes apart. And remember less is more with thermal film.

well, better be safe than sorry. i would just wipe out the old thermal and put some new thermal on it. it only takes you 5 mins to do that. and you dont wanna fry your $400 CPU.

DO NOT try to start a computer without any paste or pad on the CPU. I did a few years ago and I fried it.
