ps2/xbox deluxe pad model: 505-1054

I bought a pad for the ps2 about 3 yrs ago. a friend borrowed it and when I got it back it no longer had all the cords and the game disc was gone. Is there anyway to identify this pad and get replacement parts? all it says is "ps2/xbox deluxe pad model: 505-1054" on the back, the front had colored squares with "stay cool" in the center square and "show me your best" on the surounding 8 squares.
